Who can join?

All Care Plans are available to everyone, including new patients. Some plans require a pre-assessment. If any treatment is required, this will need to be completed before joining the plan.

How do I pay?

Payment is by monthly direct debit. .

Terms and Conditions

Maintenance Plan Agreement Terms

1. Fees Payable by the Patient or the Payer

The Patient will, or if a Payer is named on the Application Form, will procure that the Payer will pay to Ibrahim & Adam Ltd:

1.1

The monthly fee specified on the Application Form, payable monthly in advance for the dental services listed in Clause 2.3 ("the Services").

1.2

The registration fee specified on the Application Form, payable upon signing the Application.

1.3

Payments will be collected by Direct Debit or through a third-party Scheme Administrator appointed by Ibrahim & Adam Ltd.

  • Payments are taken monthly.
  • The registration fee and first monthly payment may be collected up to two months after commencement due to administration requirements.
  • This may result in a double payment covering:
    • Registration fee
    • First month's payment
    • Second month's payment

1.4

Ibrahim & Adam Ltd may, at its sole discretion, agree in writing to accept payment by another method. Unless otherwise agreed in writing, these Terms and Conditions continue to apply.


2. Patient's Oral Fitness and Services

2.1

Ibrahim & Adam Ltd offers the following plans:

  • Essential Care
  • Essential Care Plus
  • Child Care
  • Gum Care
  • Ortho Care
  • Premium Care

The selected scheme is specified on the Application Form.

For:

  • Essential Care
  • Essential Care Plus
  • GumCare

There is no requirement for the Patient to be orally fit. Services commence after four monthly payments have been made.

For:

  • Premium Care
  • Child Care
  • Ortho Care

The Patient must be orally fit before Services begin and is required to undergo an examination.

2.2

In consideration of the Patient complying with this Agreement, Ibrahim & Adam Ltd agrees to provide the Services at:

90 Main Street, Menston, LS29 6HY

during normal opening hours, either by the named Dentist or another suitably qualified practitioner working under contract or locum agreement.

2.3 Services Included

(a) Essential Care

Includes:

  • One routine 10-point dental examination per year
  • All X-rays
  • Advice on improving or maintaining dental health
  • One routine hygiene appointment per year
  • Preventative dental advice
  • Emergency dental assessments
  • 15% discount on additional treatment

(b) Essential Care Plus

Includes everything in Essential Care plus:

  • Two routine dental examinations per year
  • Two routine hygiene appointments per year

(c) Child Care

For children up to 17 years of age.

Includes:

  • One routine dental examination per year
  • One routine hygiene treatment per year
  • All X-rays
  • Oral health advice
  • Preventative dental advice
  • Emergency dental assessments
  • All clinically necessary treatment up to £2200 per year

(d) Gum Care

Includes everything in Essential Care Plus plus:

  • Up to two additional routine hygiene appointments per year
  • Up to two air polish treatments per year

    (e) Ortho Care

    Includes:

    • One set of upper and lower clear removable retainers
    • One minor repair of a bonded retainer

    (f) Premium Care

    • Two examinations per year
    • Two routine hygiene treatments per year
    • All X rays
    • Up to £2200 of dental treatment per year

     


    3. Responsibilities of the Patient

    3.1

    The Patient (or Payer) must pay all fees due under this Agreement. Failure to pay may result in immediate suspension or termination of the Agreement and Services.

    3.2

    The Patient is responsible for arranging appointments.

    No refunds are available for unused Services, and unused Services cannot be carried forward.

    3.3 Missed Appointments

    Patients must attend booked appointments or pay the missed appointment fee.

    Current fee:

    £20.00 per 20 minutes of surgery time.

    This fee will not be charged if the appointment is cancelled and rearranged at least 24 hours in advance.

    3.4

    Patients must:

    • Attend regular examinations
    • Receive recommended treatment
    • Inform the Dentist of any injury or change affecting oral health

    Failure to do so may result in charges for treatment that would otherwise have been included.

    3.5

    Patients must immediately notify Ibrahim & Adam Ltd of any changes to contact or bank details.

    3.6

    Where a Payer is named, the Patient confirms that the Payer has agreed:

    • To pay the required fees
    • To the use of their personal information under this Agreement

    4. Queries, Complaints and Liability

    4.1

    Complaints should be submitted in writing to:

    Complaints will be handled fairly and promptly.

    4.2

    Nothing in this Agreement limits or excludes liability for death or personal injury caused by negligence.

    4.3

    Subject to Clause 4.2, Ibrahim & Adam Ltd's maximum liability is limited to 100% of the total fees paid during the previous 12 months.

    4.4

    Subject to Clauses 4.2 and 4.3, Ibrahim & Adam Ltd accepts no liability for negligence by the Dentist.


    5. Changes to this Agreement

    5.1

    Ibrahim & Adam Ltd may change:

    • Monthly fees
    • Services
    • Scheme details
    • Terms of this Agreement

    by providing at least one month's written notice.

    5.2

    Notice is deemed served 48 hours after being sent by email to the Patient's registered email address.


    6. Termination

    6.1

    Either party may terminate this Agreement by giving at least one month's written notice.

    If changes are made under Clause 5.1, the Patient may terminate within one month of receiving notice.

    6.2 Upon Termination

    • All Services cease immediately.
    • No refund is payable for unused Services.

    7. Change of Dentist

    7.1

    The Patient may request a different dentist contracted with Ibrahim & Adam Ltd.

    Approval is subject to written agreement by both Ibrahim & Adam Ltd and the new Dentist.

    7.2

    Treatment received outside Ibrahim & Adam Ltd is not covered by this Agreement.

    7.3

    No third party has any right to enforce this Agreement.

    Ibrahim & Adam Ltd may transfer the benefit of this Agreement to another party.

    The Patient may not transfer their rights.


    8. General Provisions

    8.1 Entire Agreement

    For the purposes of this Agreement, Monthly Dental Plans include:

    • Essential Care
    • Essential Care Plus
    • Child Care
    • Gum Care
    • Ortho Care
    • Premium Care

    The following documents together constitute the entire Agreement:

    • These Terms and Conditions
    • The Application Form
    • Practice Leaflets
    • Any written terms supplied before entering the Agreement

    If there is any inconsistency, the order of precedence is:

    1. Other written terms
    2. Application Form
    3. These Terms and Conditions
    4. Practice Leaflet

    This Agreement replaces any previous agreement relating to these Services.

    8.2 Governing Law

    This Agreement is governed by the laws of England and Wales, and both parties submit to the exclusive jurisdiction of the English Courts.
